First, contact the insurance company and determine whether your premium is simply late, whether the policy is still within its grace period, or whether the coverage has officially terminated.

In Pennsylvania, an individual Medicare Supplement policy must provide at least a 30-day grace period for premiums due after the first premium. The policy remains in force during that grace period. If the grace period has already expired, the company may still allow reinstatement, but it may require overdue premiums, a reinstatement application, or evidence of insurability.

Do not immediately submit an application to another company until you understand the status of your existing policy. Outside your Medigap Open Enrollment Period or a guaranteed-issue situation, another insurance company may use medical underwriting and may decline the application.

Can I Reinstate My Medicare Supplement Policy?

Possibly.

Whether you can reinstate your Medicare Supplement policy may depend on:

  • Whether the policy is still within its grace period
  • How long ago the premium was due
  • Whether the policy has officially terminated
  • The insurance company’s reinstatement procedures
  • Whether the company requires a reinstatement application
  • Whether evidence of insurability is required
  • Whether all overdue premiums are paid

In some cases, the company may accept the overdue premium and restore the policy. In other situations, the company may require an application and review before approving reinstatement.

Submitting a payment does not necessarily prove that the policy is active again. Ask the company for written confirmation of the policy’s status.

First Determine Whether the Policy Is Actually Lapsed

People commonly use the word “lapsed” whenever they receive a late-payment or cancellation notice. However, the policy may not have terminated yet.

Your policy may be in one of three different stages:

The Premium Is Late

The payment due date has passed, but the policy may still be active.

The Policy Is Within Its Grace Period

The payment is overdue, but the grace period has not expired. In Pennsylvania, coverage remains in force during the required grace period.

The Policy Has Terminated

The grace period has ended and the insurance company has officially ended the coverage for nonpayment.

These situations are not the same. Before making another decision, ask the company to tell you exactly which one applies.

How the Pennsylvania Medicare Supplement Grace Period Works

Pennsylvania Medicare Supplement policies must provide a grace period of at least 30 days for premiums due after the first premium.

During that grace period:

  • The policy remains in force
  • The overdue premium is still owed
  • You should pay the amount due as quickly as possible
  • You should confirm that the payment was properly applied
  • You should request confirmation that the policy remains active

The grace period does not mean that premiums can be ignored indefinitely. Once the grace period ends, the insurance company may terminate the policy for nonpayment.

What Happens After the Grace Period Ends?

Once a Medicare Supplement policy has officially terminated, reinstatement may become more complicated.

Pennsylvania policies must contain a provision describing reinstatement after nonpayment. Depending on the policy and insurance company, the carrier may require:

  • Payment of all overdue premiums
  • A reinstatement application
  • Updated billing information
  • Medical questions
  • Evidence of insurability
  • Review and approval by the insurance company

Pennsylvania’s policy requirements specifically permit an insurer to require an application or evidence of insurability when reinstatement is requested.

Do not assume that mailing a check, making an online payment, or providing new bank information automatically reinstates the coverage. Confirm the decision directly with the insurance company.

What Should I Ask the Insurance Company?

Call the customer-service number shown on your premium notice, insurance card, or policy documents.

Ask these questions:

  1. Is my Medicare Supplement policy currently active?
  2. Am I still within the grace period?
  3. What is the exact date the policy terminated or will terminate?
  4. What amount must I pay to bring the policy current?
  5. Will you accept the overdue premium without a reinstatement application?
  6. Is a reinstatement form required?
  7. Will I need to answer medical questions?
  8. How will claims incurred during this period be handled?
  9. When will I receive written confirmation of reinstatement?

Write down the representative’s name, the date and time of the call, and any confirmation number provided.

Keep copies of:

  • Cancellation notices
  • Late-payment notices
  • Bank statements
  • Cancelled checks
  • Online payment confirmations
  • Emails from the insurance company
  • Reinstatement forms
  • Written approval notices

These records may become important if there is disagreement about the payment or termination date.

Why Do Medicare Supplement Policies Lapse?

Nonpayment is the most common reason, but it is not always intentional.

A payment may be missed because:

  • A credit or debit card expired
  • A bank account was closed or changed
  • An automatic withdrawal was rejected
  • The premium increased and the old amount was submitted
  • A mailed bill went to an outdated address
  • The policyholder was hospitalized
  • Someone else had been handling the bills
  • A bank replaced an account after suspected fraud
  • The policyholder believed automatic payments were still active

After resolving the immediate problem, verify the billing method, mailing address, email address, and automatic-payment information on the policy.

Should I Apply for a New Medicare Supplement Policy?

Not until you know whether the existing policy can be saved.

Reinstating an established policy may be simpler and less risky than applying for a completely new one.

A new Medicare Supplement application may require medical underwriting when you are outside your six-month Medigap Open Enrollment Period and do not have a guaranteed-issue right. The new company may approve the application, request additional information, charge a different permitted premium, or decline the application.

A lapse caused by missed premiums should not be treated as proof that another company must accept you without underwriting.

Determine the following before applying elsewhere:

  • Whether reinstatement remains available
  • Whether a guaranteed-issue right applies
  • Whether your health history meets another company’s underwriting guidelines
  • Whether there would be a gap between policies
  • Whether the proposed replacement is actually approved

Never cancel or abandon an active policy merely because another application has been submitted.

What If I Have Health Problems?

Health problems do not necessarily mean that no options are available, but they can affect which companies may consider an application.

Underwriting guidelines are not identical among Medicare Supplement companies. One company may view a medical condition, treatment, or prescription differently from another.

An independent Medicare broker can review the available underwriting questions before an application is submitted. However, only the insurance company can make the final approval or reinstatement decision.

What If the Insurance Company Made an Error?

Ask the company to investigate the payment and billing history.

Provide copies of any documentation showing that:

  • The premium was paid
  • An automatic withdrawal should have occurred
  • The payment was sent before the deadline
  • The company used an incorrect address
  • The termination notice contained incorrect information
  • A reinstatement payment was accepted

If you believe a Pennsylvania insurance company cancelled the policy incorrectly or failed to provide proper notice, you may contact the Pennsylvania Insurance Department for assistance or file a consumer complaint.

How Can an Independent Medicare Broker Help?

An independent Medicare broker cannot force an insurance company to reinstate a terminated policy.

A broker can, however, help you:

  • Identify the correct insurance-company department
  • Understand the difference between a late payment and termination
  • Review the company’s response
  • Determine whether reinstatement is still being considered
  • Review possible guaranteed-issue rights
  • Compare underwriting requirements if new coverage is necessary
  • Avoid cancelling coverage prematurely
  • Coordinate effective dates if a replacement is approved

The first objective should be protecting or restoring your existing coverage whenever that remains possible.

The Bottom Line

A missed Medicare Supplement payment does not always mean that coverage has been permanently lost.

The policy may still be active, within its grace period, or eligible for reinstatement. The sooner you contact the insurance company, pay any amount due, and obtain written confirmation, the better.

Do not assume that the policy is active, and do not assume that you must immediately replace it. Find out exactly what happened before making another coverage decision.
