Both plans require you to pay the annual Medicare Part B deductible. Neither Plan G nor Plan N covers that deductible for people who became eligible for Medicare on or after January 1, 2020.

What Does Medicare Supplement Plan G Cover?

Plan G is one of the most comprehensive Medicare Supplement plans available to people newly eligible for Medicare.

After you pay the annual Part B deductible, Plan G generally covers your remaining Medicare-approved coinsurance and copayments for covered services. It also covers Part B excess charges when they legally apply.

Plan G may appeal to you if you:

  • Want highly predictable healthcare expenses
  • Prefer not to deal with office or emergency-room copays
  • Regularly receive care in states where excess charges are permitted
  • Are comfortable paying more each month for fewer out-of-pocket bills
  • Value simplicity more than maximizing potential premium savings

Plan G does not provide broader doctor access than Plan N. Both plans supplement Original Medicare and allow you to visit any healthcare provider nationwide who accepts Medicare.

What Does Medicare Supplement Plan N Cover?

Plan N covers the same major hospital benefits as Plan G, including the Medicare Part A deductible and Part A hospital coinsurance.

For Medicare Part B services, Plan N pays the Part B coinsurance except for:

  • A copayment of up to $20 for some office visits
  • A copayment of up to $50 for an emergency-room visit that does not result in inpatient admission
  • Medicare Part B excess charges when they legally apply

The $50 emergency-room copay is waived when the emergency-room visit results in an inpatient hospital admission.

Plan N may appeal to you if you:

  • Want a lower monthly premium
  • Do not mind paying occasional copays
  • Rarely visit the doctor
  • Live in a state that prohibits excess charges
  • Believe the annual premium savings will exceed your copays
  • Want strong coverage for major Medicare-approved expenses

The Biggest Difference Is Usually the Premium

The most meaningful difference between Plan G and Plan N is often the monthly premium—not excess charges.

Suppose Plan N costs $35 less per month than Plan G. That produces annual premium savings of:

$35 × 12 months = $420 per year

If you have six office visits during the year and each visit results in a $20 copay, you would pay:

6 × $20 = $120 in copays

In this example, Plan N would still save approximately:

$420 − $120 = $300 for the year

The exact numbers will depend on your age, location, insurance company, discounts, and healthcare use. Not every office visit necessarily produces the full $20 copay, either.

The right way to compare the plans is to calculate the annual premium difference and decide whether Plan G’s additional benefits are reasonably likely to repay that difference.

Do Plan N Copays Apply to Every Medical Service?

No. The Plan N copay is not a flat $20 charge for every interaction with a medical provider.

The copay can apply to certain office visits after the Part B deductible has been satisfied. Other Medicare-covered Part B services may not result in a Plan N copay.

Plan N also does not impose copays for inpatient hospital stays. Its hospital benefits are substantially the same as Plan G’s benefits.

This is why simply multiplying every medical appointment by $20 may overstate what someone will actually spend with Plan N.

What Are Medicare Part B Excess Charges?

A Medicare Part B excess charge may occur when a doctor accepts Medicare patients but does not accept Medicare assignment. A non-participating provider may be permitted to charge up to 15% above Medicare’s approved amount for certain Part B services.

Plan G covers qualifying Part B excess charges. Plan N does not.

However, excess charges are prohibited in several states. If you receive covered services in a state where excess charges are prohibited, a provider cannot impose an excess charge merely because your permanent residence is elsewhere.

The reverse is also important: living in a state that prohibits excess charges does not necessarily protect you when receiving care in another state where they are permitted.

Non-Participating Providers Are Not the Same as Opt-Out Providers

A Medicare non-participating provider still participates in the Medicare program but has not agreed to accept Medicare assignment for every covered service. This is the situation in which an excess charge may potentially apply, subject to state law.

A provider who has opted out of Medicare is different. An opt-out provider generally uses a private contract with the patient and is not limited to Medicare’s approved amount.

Plan G’s excess-charge benefit does not turn private-contract charges from an opt-out provider into Medicare-covered expenses. Plan G and Plan N are designed to supplement Medicare-covered services; they do not create Medicare coverage when a provider has opted out of the program.

Before scheduling non-emergency care, you can ask:

  • Do you accept Medicare?
  • Do you accept Medicare assignment?
  • Have you opted out of Medicare?
  • Will I be asked to sign a private contract?

Do Plan G and Plan N Have Different Doctor Networks?

No. Neither Plan G nor Plan N uses a proprietary doctor network.

With either plan, you can generally receive Medicare-covered care from any doctor or hospital in the United States that accepts Medicare. Referrals are not required by the Medicare Supplement policy.

Plan G does not provide a larger network or greater freedom to travel than Plan N. The provider-access rules come from Original Medicare and are the same for both plans.

The travel-related distinction involves excess charges. Plan G covers them when they apply, while Plan N does not.

Do Plan G and Plan N Cover Foreign Travel?

Both plans include the same limited foreign-travel emergency benefit.

After a $250 calendar-year deductible, the benefit generally pays 80% of qualifying emergency expenses during the first 60 days of a trip, subject to a $50,000 lifetime limit.

This is not comprehensive international health insurance. People who regularly travel outside the United States may want to consider separate travel medical coverage.

When Is Plan N Usually the Better Value?

Plan N can provide better value when its premium savings comfortably exceed the copays and other expenses you are likely to incur.

It may be particularly attractive when:

  • The monthly premium difference is substantial
  • You have relatively few office visits
  • You live in or primarily receive care in a state prohibiting excess charges
  • Your doctors accept Medicare assignment
  • You are comfortable handling occasional small bills
  • You want to reduce the amount you pay to the insurance company every month

Plan N is not inferior hospital coverage. It provides strong protection against the major hospital expenses left by Original Medicare.

When May Plan G Be Worth the Higher Premium?

Plan G may be the better choice when predictability is more important to you than potential savings.

It may be worth considering if:

  • The premium difference between Plan G and Plan N is small
  • You expect frequent office visits
  • You dislike receiving medical bills
  • You receive care from non-participating providers
  • You frequently travel to states where excess charges are permitted
  • You are willing to pay more for simpler expense management

Plan G can also provide peace of mind for someone who does not want to evaluate whether potential copays and excess charges will outweigh Plan N’s premium savings.

Compare the Insurance Companies as Well as the Plans

Medicare Supplement benefits are standardized. A Plan G issued by one company provides the same standardized medical benefits as a Plan G issued by another company. The same is true for Plan N.

However, insurance companies can charge very different premiums for identical standardized benefits.

You should compare:

  • Current monthly premium
  • Available household discounts
  • The company’s rate history
  • Financial strength
  • Customer service
  • Application and underwriting requirements
  • How the policy is priced as you grow older

Choosing Plan G or Plan N is only half the decision. Choosing a competitively priced company can make an equally important difference over time.

Can You Switch Between Plan G and Plan N Later?

Possibly—but approval is not always guaranteed.

Outside your Medigap Open Enrollment Period or another guaranteed-issue situation, an insurance company may require medical underwriting. Your health history can affect whether the company approves your application.

Do not assume you can automatically purchase Plan G later if your health changes or you begin using more medical care. Before replacing an existing Medicare Supplement policy, make sure the new policy has been approved and you understand when the new coverage begins.

The Bottom Line on Plan G vs Plan N

Plan G vs Plan N comes down to how you prefer to pay for your healthcare.

Plan G generally charges a higher premium in exchange for fewer out-of-pocket bills. Plan N generally charges a lower premium but leaves you responsible for certain office and emergency-room copays and any legally permitted Part B excess charges.

For many people, Plan N provides excellent coverage and better overall value. For others, Plan G’s predictability is worth the additional premium.

The best decision should be based on the actual premium difference in your area, your doctors, your expected healthcare use, and how strongly you value predictable expenses.
