Neither Plan G nor Plan N covers the annual Medicare Part B deductible for people who became eligible for Medicare on or after January 1, 2020.

How Pennsylvania Changes the Plan G vs Plan N Comparison

National comparisons often make Medicare Part B excess charges sound like one of the biggest differences between Plan G and Plan N.

That is not the most useful way to compare the plans in Pennsylvania.

Under Pennsylvania’s Health Care Practitioner Medicare Fee Control Act, healthcare practitioners cannot balance bill Medicare beneficiaries above the Medicare-approved amount for Medicare-covered services received in Pennsylvania.

That means a Pennsylvania provider cannot impose a Medicare Part B excess charge simply because the provider is non-participating or does not accept assignment for a particular claim.

Plan G technically includes excess-charge coverage, while Plan N does not. However, that Plan G benefit generally has nothing to pay when the covered service is received in Pennsylvania.

For most Pennsylvania residents, the more important comparison is:

  • How much more Plan G costs each month
  • How many Plan N office copays you are likely to pay
  • Whether you frequently receive care outside Pennsylvania
  • Whether avoiding occasional bills is worth the higher premium

The Biggest Practical Difference Is Premiums Versus Copays

Plan G usually has a higher monthly premium. In exchange, it generally covers the remaining Medicare-approved Part B coinsurance after you satisfy the annual Part B deductible.

Plan N typically has a lower premium, but you may pay:

  • Up to $20 for some office visits
  • Up to $50 for an emergency-room visit that does not result in inpatient admission

The $50 emergency-room copay does not apply when the emergency-room visit results in an inpatient hospital admission.

The key is to compare the annual premium difference with the out-of-pocket costs you could reasonably expect under Plan N.

Suppose Plan N costs $45 less per month than Plan G:

$45 × 12 months = $540 in annual premium savings

If you have eight office visits and pay the full $20 copay for each one:

8 × $20 = $160 in copays

Your remaining estimated savings would be:

$540 − $160 = $380 for the year

This is only an illustration. Premiums vary by age, ZIP code, insurance company, household discount, tobacco status, and other rating factors. Not every medical appointment necessarily results in the full $20 Plan N copay.

What Does Plan G Cover in Pennsylvania?

Plan G is one of the most comprehensive Medicare Supplement plans available to people newly eligible for Medicare.

After you pay the annual Medicare Part B deductible, Plan G generally covers your remaining Medicare-approved coinsurance and copayments for covered services.

Plan G also covers Medicare Part B excess charges when they legally apply. However, Pennsylvania prohibits practitioners from imposing those charges for covered services received in the state.

Plan G may appeal to Pennsylvania residents who:

  • Want highly predictable healthcare expenses
  • Prefer not to receive office or emergency-room copay bills
  • Expect frequent office visits
  • Regularly receive medical care outside Pennsylvania
  • Are comfortable paying more each month for simplicity
  • Find only a small premium difference between Plan G and Plan N

Plan G does not provide a larger doctor network than Plan N. The two plans have the same Original Medicare provider access.

What Does Plan N Cover in Pennsylvania?

Plan N covers the same major hospital benefits as Plan G, including the Medicare Part A deductible, hospital coinsurance, and skilled nursing facility coinsurance.

For Medicare Part B services, Plan N pays the Part B coinsurance except for:

  • A copayment of up to $20 for some office visits
  • A copayment of up to $50 for an emergency-room visit that does not result in inpatient admission
  • Part B excess charges when they legally apply

Because Pennsylvania prohibits excess charges for covered services received in the state, the practical difference for many Pennsylvania residents is primarily Plan N’s copays.

Plan N may appeal to you if you:

  • Want to reduce your monthly premium
  • Do not mind occasional small copays
  • Believe the annual savings will exceed your copays
  • Primarily receive your healthcare in Pennsylvania
  • Use doctors who accept Medicare
  • Want strong protection from major hospital expenses
  • Prefer keeping more money instead of paying a higher insurance premium every month

Plan N is not inferior hospital coverage. It provides the same standardized major hospital benefits included with Plan G.

Do Plan G and Plan N Have the Same Doctor Access?

Yes. Plan G and Plan N provide the same nationwide Original Medicare provider access.

Neither standard Plan G nor standard Plan N uses a proprietary doctor network. With either plan, you can generally receive Medicare-covered care from any doctor or hospital in the United States that accepts Medicare.

Plan G does not provide:

  • A larger network
  • More doctors
  • Additional hospital access
  • Greater freedom to travel
  • Different referral rules

Those provider-access rights come from Original Medicare and are the same with both plans.

Plan G’s travel-related distinction is that it covers qualifying excess charges when they occur in a state where those charges are permitted. Plan N does not cover them.

What Happens When a Pennsylvania Resident Receives Care Out of State?

Pennsylvania’s balance-billing protection generally applies to services received in Pennsylvania. Your Pennsylvania residency does not necessarily protect you when you receive care in another state.

If you receive Medicare-covered care in a state where excess charges are permitted, a non-participating provider may be allowed to charge more than Medicare’s approved amount, subject to Medicare’s limiting charge.

In that situation:

  • Plan G covers qualifying Part B excess charges
  • Plan N leaves you responsible for the excess charge

This does not mean every Pennsylvania resident who travels should automatically choose Plan G. Most Medicare providers accept assignment, and the premium difference may still be greater than your realistic excess-charge exposure.

If you frequently schedule medical care outside Pennsylvania, ask the provider:

  • Do you accept Medicare?
  • Do you accept Medicare assignment?
  • Are you a non-participating Medicare provider?
  • Have you opted out of Medicare?
  • Will I be asked to sign a private contract?

Non-Participating Providers and Opt-Out Providers Are Different

A non-participating Medicare provider has not agreed to accept Medicare assignment for every covered service. The provider can still treat Medicare beneficiaries and may accept assignment on a claim-by-claim basis.

In states permitting excess charges, a non-participating provider may charge more than the Medicare-approved amount, subject to Medicare’s limiting charge.

Pennsylvania law prevents practitioners from imposing that excess amount for covered services received in Pennsylvania. However, this does not mean every Pennsylvania provider is officially a participating Medicare provider.

An opt-out provider is different.

A provider who has opted out of Medicare generally:

  • Does not submit regular claims to Medicare
  • Uses a private contract with the patient
  • Is not limited to Medicare’s approved amount under that private contract
  • Requires the patient to agree to pay personally for the services

Except for certain emergency or urgently needed services, Medicare generally does not pay for care provided under an opt-out private contract.

Plan G’s excess-charge benefit does not cover unlimited private-contract fees from an opt-out provider. Plan G and Plan N supplement Medicare-covered expenses; they do not create Medicare coverage for private-contract services.

When Is Plan N Usually the Better Value in Pennsylvania?

Plan N can provide excellent value when its annual premium savings comfortably exceed the copays you are likely to pay.

It may be the stronger choice when:

  • Plan N is substantially less expensive than Plan G
  • You have relatively few office visits
  • You primarily receive medical care in Pennsylvania
  • You are comfortable paying occasional copays
  • Your doctors accept Medicare
  • You want to reduce your monthly insurance expenses
  • You understand that major hospital benefits are substantially the same

Even people who regularly visit doctors may save money with Plan N. The decision should be based on actual math, not merely on whether you see doctors frequently.

If Plan N saves you $600 per year, it would take 30 office visits at the full $20 copay to equal that savings. Not every visit will necessarily produce the maximum copay.

When May Plan G Be Worth the Higher Premium?

Plan G may be worthwhile when predictability and simplicity are more important to you than maximizing potential savings.

Consider Plan G when:

  • The premium difference between Plan G and Plan N is small
  • You strongly dislike receiving medical bills
  • You expect numerous office visits
  • You frequently use emergency rooms without being admitted
  • You regularly schedule care outside Pennsylvania
  • You use non-participating providers in states permitting excess charges
  • You prefer paying more monthly for fewer out-of-pocket expenses

Plan G can offer peace of mind, but peace of mind has a price. The question is whether the additional premium is reasonable for the benefits you receive.

Compare the Insurance Companies Too

Choosing between Plan G and Plan N is only part of the decision.

Medicare Supplement benefits are standardized. Every standard Plan G provides the same basic medical benefits, regardless of the insurance company. The same is true for Plan N.

Insurance companies can still differ significantly in:

  • Monthly premiums
  • Household discounts
  • Rate-increase history
  • Financial strength
  • Customer service
  • Underwriting requirements
  • Pricing methods
  • Premiums at older ages

An expensive Plan N from one company could potentially cost more than a competitively priced Plan G from another company. Compare the actual rates available to you instead of assuming one plan letter will always be cheaper.

Can You Switch Between Plan G and Plan N Later?

Possibly, but approval is not always guaranteed.

During your six-month Medigap Open Enrollment Period, you generally have the right to purchase any Medicare Supplement plan available to you without medical underwriting.

Outside that period or another guaranteed-issue situation, an insurance company may ask health questions and review your medical history before approving a new policy.

Do not assume you can automatically switch from Plan N to Plan G later if your health changes or you begin visiting doctors more frequently.

Before canceling an existing Medicare Supplement policy:

  • Apply for the new coverage
  • Wait for written approval
  • Confirm the effective date
  • Review the new premium
  • Make sure you understand any underwriting decision

Your existing policy should remain active until the replacement coverage has been approved and properly scheduled to begin.
