What Is the Difference Between Medicare Advantage and Medicare Supplement?

Once you have Medicare Parts A and B, you generally choose between two main ways of receiving and managing your Medicare coverage.

With Original Medicare and a Medicare Supplement plan, the federal Medicare program remains your primary coverage. The Medicare Supplement policy helps pay certain deductibles, coinsurance, and copayments left by Original Medicare.

With Medicare Advantage, you receive your Medicare Part A and Part B benefits through a Medicare-approved private insurance plan. Most Medicare Advantage plans also include prescription drug coverage and may offer additional benefits.

You cannot use a Medicare Supplement policy to pay Medicare Advantage copays or deductibles. Medicare Supplement coverage is designed to work with Original Medicare, not Medicare Advantage.

Medicare Advantage at a Glance

• A private insurance plan administers your Medicare Part A and Part B benefits
• Many plans have a low or $0 additional monthly premium
• HMO or PPO provider networks may apply
• You may pay copays or coinsurance when receiving medical care

Medicare Supplement at a Glance

• A separate Medicare Part D plan is generally needed

How Original Medicare With a Medicare Supplement Works

Original Medicare includes:

  • Medicare Part A for hospital-related coverage
  • Medicare Part B for physician services, outpatient care, preventive services, testing, and other medically necessary care

Original Medicare generally allows you to receive covered care from any doctor or hospital in the United States that accepts Medicare.

However, Original Medicare leaves beneficiaries responsible for deductibles and coinsurance. It also has no annual out-of-pocket maximum unless you have additional coverage.

A Medicare Supplement plan, also called Medigap, helps pay some or most of those remaining expenses. The benefits depend on the standardized plan letter you select.

Most people who choose this approach also enroll in a separate Medicare Part D prescription drug plan.

How Medicare Advantage Works

Medicare Advantage is also called Medicare Part C.

You remain enrolled in Medicare, but a private insurance company administers your Medicare-covered Part A and Part B services. Medicare Advantage plans must cover the medically necessary services covered by Original Medicare, although the rules and out-of-pocket costs can differ.

Depending on the plan, you may have:

  • Doctor and hospital networks
  • Copays for office visits
  • Copays or coinsurance for hospital care
  • Prior-authorization requirements
  • Referral requirements
  • An annual out-of-pocket maximum
  • Prescription drug coverage
  • Dental, vision, hearing, or fitness benefits

Some Medicare Advantage plans have a $0 additional monthly premium. However, you must generally continue paying your Medicare Part B premium, and you may owe copays or coinsurance when you use medical services.

Doctor and Hospital Access

Provider access is one of the largest differences between Medicare Advantage and Medicare Supplement coverage.

With Medicare Supplement

You can generally use any doctor or hospital nationwide that accepts Original Medicare.

A standard Medicare Supplement policy does not ordinarily have its own provider network. Your doctor does not need a separate contract with the Medicare Supplement insurance company.

This can be valuable if you:

  • Travel frequently
  • Spend part of the year in another state
  • Want access to specialists outside your local area
  • Prefer not to worry about annual network changes

With Medicare Advantage

Many Medicare Advantage plans use HMO or PPO networks.

An HMO generally requires members to receive non-emergency care from participating providers. PPO plans may provide some out-of-network coverage, but the member may pay more.

Before enrolling, confirm that your doctors, hospitals, specialists, pharmacies, and other providers participate in the specific plan. Do not rely solely on the insurance company’s name because each plan can have a different network.

Monthly Premiums and Medical Expenses

Medicare Advantage plans often have lower additional monthly premiums than Medicare Supplement plans.

That does not necessarily mean the total annual cost will always be lower.

With Medicare Advantage, you may pay as you receive care through:

  • Office-visit copays
  • Specialist copays
  • Outpatient testing costs
  • Hospital copays or coinsurance
  • Physical therapy copays
  • Skilled-nursing costs
  • Ambulance charges

Every Medicare Advantage plan has an annual maximum for covered Part A and Part B out-of-pocket expenses. The amount varies by plan.

With a Medicare Supplement, you usually pay a separate monthly premium. In exchange, the policy may cover most of the Medicare-approved cost sharing that Original Medicare leaves behind.

This can make medical expenses more predictable, particularly with comprehensive plans such as Plan G.

The comparison is therefore not simply low premium versus high premium. It is also:

  • Lower monthly cost with more pay-as-you-go expenses
  • Higher monthly cost with greater predictability

Prescription Drug Coverage

Most Medicare Advantage plans include Medicare Part D prescription drug coverage.

If you choose Original Medicare with a Medicare Supplement, you will generally need to purchase a separate Part D plan if you want outpatient prescription coverage.

Drug coverage should be compared separately using:

  • Your medications
  • Dosages
  • Preferred pharmacies
  • Plan formularies
  • Deductibles
  • Copays and coinsurance
  • Plan restrictions

A plan with attractive medical benefits may still be a poor choice if it does not cover your prescriptions affordably.

Dental, Vision, Hearing, and Fitness Benefits

Original Medicare and standardized Medicare Supplement plans generally do not cover routine dental care, routine vision examinations, hearing aids, or fitness memberships.

Many Medicare Advantage plans advertise these additional benefits.

These extras can provide value, but examine the details carefully:

  • What is the annual benefit limit?
  • Which providers can be used?
  • Are more expensive services excluded?
  • Is prior approval required?
  • Does the benefit cover the service completely or only provide a discount?
  • Can the benefit change in a future plan year?

Do not select medical coverage solely because of a dental allowance or gym membership. Doctor access, hospital coverage, medical costs, and plan rules are usually more important.

Prior Authorization and Referrals

Original Medicare usually does not require prior authorization for most covered services, although there are limited exceptions.

Medicare Advantage plans may require prior authorization before covering certain tests, procedures, medical equipment, rehabilitation, or other care.

Some Medicare Advantage plans may also require referrals before seeing specialists. The rules depend on the specific plan.

Prior authorization does not mean a service will automatically be denied. It means the plan may require the provider to obtain approval before the service is covered.

People with ongoing or complex medical needs should pay particular attention to these rules when comparing plans.

Travel and Living in More Than One State

A standard Medicare Supplement policy generally works nationwide with providers who accept Original Medicare.

This often makes Medicare Supplement attractive to people who:

  • Travel regularly
  • Visit family in other states
  • Spend winters in another part of the country
  • Maintain more than one residence
  • Want access to major medical centers outside their local area

Medicare Advantage plans must cover emergency and urgently needed care when you are outside the plan’s service area. Routine non-emergency care may be limited or cost more outside the network, depending on the plan.

Someone who travels frequently should review out-of-area coverage before joining Medicare Advantage.

Who May Prefer Medicare Advantage?

Medicare Advantage may be worth considering if you:

  • Prioritize a lower monthly premium
  • Are comfortable using a provider network
  • Have confirmed that your doctors and hospitals participate
  • Prefer prescription coverage bundled with medical coverage
  • Value included dental, vision, hearing, or fitness benefits
  • Are comfortable paying copays as services are received
  • Understand the plan’s prior-authorization rules
  • Primarily receive care within one geographic area

Medicare Advantage is not automatically bad coverage. For the right person and the right local plan, it can work very well.

Who May Prefer Medicare Supplement?

Medicare Supplement may be worth considering if you:

  • Want broad nationwide provider access
  • Travel frequently or live in multiple states
  • Want fewer provider-network restrictions
  • Prefer more predictable medical expenses
  • Want to see specialists without plan-network concerns
  • Are comfortable paying a higher monthly premium
  • Do not mind purchasing separate Part D coverage
  • Want Original Medicare to remain your primary coverage

In my experience, many clients are willing to pay a higher monthly premium for the flexibility and predictability provided by Medicare Supplement coverage.

Can You Switch From Medicare Advantage to Medicare Supplement Later?

You can leave Medicare Advantage during an appropriate Medicare enrollment period and return to Original Medicare.

However, qualifying for a Medicare Supplement policy is a separate issue.

Your six-month Medigap Open Enrollment Period begins when you are at least 65 and first enrolled in Medicare Part B. During that period, an insurance company generally cannot deny your application or charge more because of pre-existing health conditions.

After that one-time period ends, medical underwriting may apply unless you qualify for a guaranteed-issue right, trial right, or another protection.

That means you should not assume you can join Medicare Advantage now and automatically purchase any Medicare Supplement plan later regardless of your health.

My View After Helping Medicare Beneficiaries Since 1985

I offer both Medicare Supplement and Medicare Advantage options, and there is no single plan that is right for everyone.

However, I frequently recommend Medicare Supplement coverage to people who value:

  • Freedom to choose Medicare providers
  • Nationwide access
  • Predictable medical expenses
  • Fewer insurance-company restrictions
  • Long-term flexibility

Medicare Advantage may be the better fit when keeping the monthly premium low is the main priority and the person is comfortable with networks, copays, and plan requirements.

The decision should be based on how the coverage works—not television advertising, brand recognition, or extra benefits alone.

Questions to Ask Before Choosing

Before enrolling, ask:

  1. Are all my doctors and hospitals available?
  2. Will I need referrals to see specialists?
  3. Which services require prior authorization?
  4. What could I pay during a year with significant medical care?
  5. Are my prescriptions covered?
  6. Are my pharmacies preferred?
  7. How does the coverage work when traveling?
  8. Do I prefer lower premiums or more predictable medical costs?
  9. Could medical underwriting affect my ability to buy Medigap later?
  10. Am I choosing the plan for its medical coverage or mainly for extra benefits?

The answers can make the correct choice much clearer.

Is Medicare Supplement better than Medicare Advantage?
