The projected Part B premium would increase by approximately 3.3 percent. The projected Part B deductible would increase by approximately 3.2 percent.

What Is the Projected 2027 Medicare Part B Premium?

The 2026 Medicare Trustees Report projects a standard 2027 Medicare Part B premium of $209.50 per month.

The standard premium is $202.90 per month in 2026. If the projection becomes final, the monthly increase would be $6.60, or $79.20 over a full year.

The $209.50 figure is not yet the final 2027 premium. CMS establishes the official Part B premium each fall after considering updated program costs, enrollment and financial information.

Some beneficiaries pay more than the standard premium because of the Income-Related Monthly Adjustment Amount, commonly called IRMAA. Other beneficiaries may be affected by Medicare’s hold-harmless provision.

What Is the Projected 2027 Medicare Part B Deductible?

The Medicare Trustees project a $292 annual Part B deductible for 2027.

The 2026 Part B deductible is $283. If the projection becomes final, the deductible would increase by $9.

The Part B deductible is paid once per calendar year before Medicare begins paying its share of most Part B-covered services.

Medicare Supplement Plans G and N do not cover the annual Part B deductible for people who became eligible for Medicare on or after January 1, 2020.

After the deductible has been satisfied, Original Medicare generally pays 80 percent of the Medicare-approved amount for covered Part B services. The beneficiary or supplemental coverage is responsible for the remaining amount.

How the 2027 Part B Changes Affect Plan G and Plan N

People with Medicare Supplement Plan G or Plan N remain responsible for the annual Part B deductible. Therefore, any increase from $283 to the projected $292 would increase their direct annual cost by $9.

After the deductible is satisfied, Plan G generally pays the remaining Medicare-approved Part B coinsurance.

Plan N generally pays the Part B coinsurance except for a copayment of up to $20 for certain office visits and up to $50 for an emergency-room visit that does not result in an inpatient admission.

Plan N does not cover Medicare Part B excess charges. Pennsylvania prohibits those charges for Medicare-covered services received within the state.

The projected increase in the Part B deductible does not change the standardized benefits provided by Plan G or Plan N.

What Are the Projected 2027 Medicare Part A Costs?

Most Medicare beneficiaries do not pay a monthly Part A premium because they or a spouse paid Medicare taxes for the required period. However, Part A includes deductibles and coinsurance when hospital or skilled-nursing facility care is received.

The Medicare Trustees currently project the following 2027 Part A costs:

  • A $1,788 inpatient hospital deductible for each benefit period
  • $447 per day for hospital days 61 through 90
  • $894 per lifetime reserve day
  • $223.50 per day for skilled-nursing facility days 21 through 100

These figures remain projections until CMS publishes the final 2027 Part A amounts.

Medicare Supplement Plans G and N generally cover the Part A hospital deductible, hospital coinsurance and skilled-nursing facility coinsurance according to their standardized benefits.

What Is Changing With Medicare Part D in 2027?

Two important Part D amounts have already been finalized for 2027.

The Maximum Part D Deductible Increases to $700

The maximum deductible permitted for a standard Medicare Part D plan increases from $615 in 2026 to $700 in 2027.

This does not mean every Part D plan will charge the full $700 deductible. Plans can use a lower deductible or apply the deductible only to particular drug tiers.

The Part D Out-of-Pocket Threshold Increases to $2,400

The annual Part D out-of-pocket threshold increases from $2,100 in 2026 to $2,400 in 2027.

After an enrollee reaches the applicable out-of-pocket threshold, the enrollee generally pays no additional cost sharing for covered Part D drugs for the remainder of the calendar year.

The threshold does not include every amount paid for prescriptions. Premiums, non-covered drugs and amounts paid outside the plan generally do not count toward the threshold.

Is the Part D Coverage Gap Returning in 2027?

No. The former Part D coverage-gap phase, commonly called the donut hole, was eliminated beginning in 2025.

CMS’s Contract Year 2027 Medicare Advantage and Part D final rule codifies the redesigned Part D structure for 2027 and future years. This includes the reduced annual out-of-pocket threshold, no enrollee cost sharing in the catastrophic phase and the Manufacturer Discount Program.

These provisions continue the redesigned Part D benefit rather than bringing back the former coverage gap.

Will Medicare Advantage Plans Change in 2027?

Individual Medicare Advantage plans can change their premiums, provider networks, drug formularies, copayments, prior-authorization requirements and supplemental benefits each year.

CMS has also finalized changes affecting Medicare Advantage and Part D Star Ratings for 2027. These changes include removing certain administrative measures and continuing the historical reward factor rather than implementing the previously developed health-equity reward.

Beneficiaries should review the Annual Notice of Change sent by their plan before the annual Medicare enrollment period. A plan that works well in 2026 should not automatically be assumed to remain unchanged in 2027.

When Will the Final 2027 Medicare Part B Costs Be Announced?

CMS normally announces the final Medicare Part A and Part B premiums, deductibles and coinsurance amounts in the fall before the new calendar year begins.

Until that announcement is published, the $209.50 Part B premium and $292 Part B deductible should be described as official Trustees’ projections—not final Medicare amounts.
